Its primary function is not flow regulation, but to ensure zero fluid passage during maintenance conditions.   However, improper installation or operation can lead to leakage, seal failure, flange distortion, and even safety risks.   The following sections summarize common installation mistakes based on engineering logic, along with their consequences.   1.
